I confirm that all information I've given regarding my health is complete and accurate. I have discussed my complaints and have had all my questions answered. I hereby consent to a chiropractic physical examination and treatment. By attending appointments my consent to examination and treatment will be automatic, unless my consent is revoked verbally. I accept that most people will experience some level of discomfort during and after sessions, which is due to the body settling down. *

If at any stage of care I'm uncomfortable, have doubts or questions then I agree to fully communicate these to my chiropractor. There is a low risk of adjustments causing permanent injury or death associated with manual manipulation or adjustments of the spine. I've had all of my questions answered in regards to this.*

Cancellation Policy

All appointments are charged in advance. If you need to change any appointments, you must give 24 hours notice. Late cancellations will be charged in full, unless the appointment can be rescheduled within 5 working days. Missed appointments are charged in full. *

To store and using your data, your consent is required. Your personal information is used to keep you informed about upcoming appointments via text and email. This includes appointment reminders and for other reasonable purposes, always acting within the limits of the General Data Protection Regulation.
